Patients are responsible for keeping the office informed of any changes to insurance, address, phone number, or pharmacy information.
Our clinics are outpatient and do not complete short-term disability or FMLA paperwork.
If a higher level of care is needed, referrals may be provided to an intensive outpatient program (IOP) or partial hospitalization program (PHP).
If you are experiencing a life-threatening emergency or urgent medical crisis, call 911 or go to the nearest emergency room immediately.
For non-urgent concerns, you may leave a voicemail or reach out via the patient portal. Messages will be returned when the office reopens.
When clinically appropriate, referrals may be provided to higher levels of care or outside treatment programs to support ongoing needs.
